Once scheduled, you will receive an email with a document called a Scope of Appointment. CMS (the government agency that regulates Medicare) requires that you complete the Scope of Appointment at least 48 prior to our appointment. If it is not received, we will need to reschedule the appointment.
For the appointment location, please choose one of the following options and type it in: on the phone, your home (type in your address), or Office (located at 110 S Gordy in El Dorado).
