If you have a spouse who wants a policy review also, go back to the calendar after filling out your information here and choose the next scheduled spot for him/her so that we will not be rushed for time.

You will receive a document called a Scope of Appointment to your phone and/or email. CMS (the government agency that regulates Medicare) requires that you complete the Scope of Appointment at least 48 prior to our appointment. If it is not received, we will need to reschedule the appointment.
